Don't know how recently it has been working, but scrolling down to the second item "Free Cable" here - http://computingondemand.com/article-5-addons-every-xbmc-user-should-have/ and http://www.maketecheasier.com/xbmc-addons-to-destroy-cable-bill/ mentions USTVNow. Not sure if you need to use a VPN to appear to be in North America for that second one.
I'm assuming you've checked iStream and Genesis's TV Show areas to make sure the individual series and episodes aren't available in higher quality?
